Currently, I work at Functional Medicine of Idaho, offering telemedicine to Idaho and Washington state patients. Here's the process of booking your first appointment. 

Step 1: Fill out the form below. After the submission, confirm your email. 

Step 2: Your contact information will be sent to one of my Care Coordinators.​

Step 3: You will receive a call from a Care Coordinator within 2-5 business days to schedule your initial consultation and begin your journey to healing! If you do not hear from anyone in that time frame, please call the clinic directly at (208) 385-7711.

FAQs

Who else is on your team?

I work with a dedicated team of providers including dietitians, health coach, trainers, and have a referral base of other specialists.

How is telemedicine billed?

We will bill your telemedicine visit to your medical insurance (if applicable), just like a standard in-person visit to our office. Our office will call you to collect your co-pay within 3 to 5 business days. It is the patient’s responsibility to ensure that telemedicine is covered under their policy. If you have questions about whether your next appointment may qualify to be a telemedicine visit or would like more information about the process, please call our office at (208) 385-7711.

Do you take insurance?

I am in-network with most Blue Cross, Regence, United Health, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans. Please call your insurance provider to guarantee coverage. Most insurances not listed above are considered out-of-network. Please see below for out of network patient options. We accept all HSA and FSA cards as a form of payment. Medicare and Medicaid are not accepted and patients will be considered as Self Pay.
